Installation instructions are found in the ReadMe.pdf, included with the distribution.
Upgrade instructions
0. Make a backup of your old simpleforum_files directory
1. Replace your old simpleforum_pro.cgi by the new one. Don’t forget to reset the permissions to 755 if necessary
2. Open the newly downloaded simpleforum_files directory, and move all its contents to your old simpleforum_files directory. Replace if necessary.
3. Go to your forum like you would normally access it and follow on-screen instructions (if any).

Thanks for emailing me the 4.6 update. I just installed it a few moments ago without a single problem. It was as smooth as can be.
It is so nice the way my customized look is now maintained with each upgrade. As you may recall, before you added the different background images functionality, I used to spend a lot of extra time re-adding my customizations after each upgrade. Those days are gone now.
I do have a question for you regarding the avatars, but I'll ask that in the proper forum.
